Sexual relationships and satisfaction can suffer during in-vitro ...

Loving couple photo by Jeremy DaccarettAn Indiana University study has found that women undergoing in-vitro fertilization report that the process of infertility treatment has many negative impacts on their sexual relationship with their partner.

Little attention has been given to the sexual dynamics of couples as they navigate infertility and treatments such as IVF, despite the important role that sex plays in a couple?s attempt to conceive a child.

?Sex is for pleasure and for reproduction, but attention to pleasure often goes by the wayside for people struggling to conceive,? said Nicole Smith, a doctoral student with the Center for Sexual Health Promotion at the IU School of Public Health-Bloomington. Smith is conducting the study in collaboration with Jody Lyne? Madeira, associate professor in the IU Maurer School of Law. ?With assisted reproductive technologies (ART), couples often report that they feel like a science experiment, as hormones are administered and sex has to be planned and timed. It can become stressful and is often very unromantic and regimented; relationships are known to suffer during the process.?

This study, which is one of the first in the United States to examine women?s sexual experiences while undergoing assisted reproductive technologies, used the Sexual Functioning Questionnaire to assess the impact of IVF treatment on couples? sexual experiences. Compared to a sample of healthy women, women undergoing IVF reported significantly less sexual desire, interest in sexual activity and satisfaction with their sexual relationship. They had more difficulty with orgasm and were more likely to report sexual problems such as vaginal pain and dryness. Similar to emotional and relationship challenges associated with assisted reproductive technologies, the sexual problems intensified as a couple?s use of ART proceeded.

When couples meet with their physicians, their sex life might not top the list of issues they want to discuss, either because of unease talking about the subject or simply because they have so many other important issues to discuss. Still, Smith and Madeira say, the doctor-patient relationship is key, and couples can be told up front about the potential sexual side effects and resources that can help. If they have issues with dryness, for example, they could be counseled on remedies such as purchasing lubricant or other sexual enhancement products. In addition to referring couples to mental health counselors, reproductive endocrinologists could also refer them to sex therapists.

?There?s just a dearth of knowledge on how infertility affects sexual behavior,? Madeira said. ?The focus is more likely to be on the social and support dimensions of the relationship, but sex is a big part of that. Just letting patients know they aren?t alone in this would be helpful.?

If more information about sexual challenges becomes available, couples might find it on their own.

?Women interested in ART are generally well-educated and tend to spend time researching these issues,? Madeira said. ?They would be very responsive to this information, and proactive.?

The study involved 270 women who completed an online questionnaire; interviews with 127 men and women using IVF to try to conceive; and interviews with 70 professionals, including physicians, nurses, mental health experts and other providers who work directly with patients.

IVF is a procedure in which mature eggs are retrieved from a woman?s ovaries and fertilized by sperm in a lab, forming embryos. The embryo(s) are then implanted in the woman?s uterus. It is considered an effective procedure but one that is used after couples try several other less invasive procedures. By the time couples begin IVF, they might have been trying to conceive for many years. Nine percent of the women in their study had been through five IVF cycles, which could take at least a year.

Prostate cancer prognosis hope

ScienceDaily (Oct. 29, 2012) ? Scientists have discovered a molecular 'tell' in laboratory experiments that could help doctors determine the severity of a patient's prostate cancer.

Cancer of the prostate -- the most common male cancer in the UK -- presents in two distinct ways: a low-risk type, which may never cause any symptoms, and a high-risk form that needs treatment to prevent it spreading to other parts of the body.

Knowing which type of prostate cancer each patient has -- some 40,000 British men per year -- is therefore essential to ensuring they receive the correct treatment.

Lead researcher Dr Angeliki Malliri, from the University of Manchester's Paterson Institute for Cancer Research, said: "Prognosis tells, or biomarkers, give doctors an indication of how a patient will fare after treatment. In prostate cancer, biomarkers that help differentiate between the low-risk and high-risk types of cancer are crucial to decide if and what type of treatment a patient needs."

The study, funded by Cancer Research UK and published in the journal Nature Cell Biology, identified the protein ?2-syntrophin as a new prognosis marker for prostate cancer. The team, part of the Manchester Cancer Research Centre (MCRC), discovered that the protein is involved in establishing tight connections between cells, which are crucial for holding them together to maintain tissue structure and prevent tumours from spreading.

Co-author Dr Natalie Mack said: "We showed that when ?2-syntrophin is lost from these cell-to-cell connections, the cells become disorganised and this is what happens in cells from prostate cancer samples, potentially helping them to spread.

"Our findings indicate that the loss of ?2-syntrophin at cell-to-cell connections in the prostate is an indicator of prostate cancer progression and patients with reduced levels of this protein at these cell-to-cell connections are more likely to have a recurrence of their cancer after treatment."

The authors say their results suggest that ?2-syntrophin is a new prognosis marker in prostate cancer and that it should be further explored to distinguish between low- and high-risk level disease. Improved understanding and use of prognosis markers is essential to help guide clinical decisions and to ensure that patients get the best type of treatment for their type of cancer.

Dr Julie Sharp, senior science information manager at Cancer Research UK, said: "To treat prostate cancer more effectively, we need to understand more about how the disease develops and how to recognise more advanced types. This research provides another piece of the puzzle and further work will confirm whether this molecule could be useful in making better predictions about prostate cancer."

NJ nuclear plant declares rare alert over high waters

NEW YORK (Reuters) - Hurricane Sandy slowed or shut a half-dozen U.S. nuclear power plants, while the nation's oldest facility declared a rare "alert" after the record storm surge pushed flood waters high enough to endanger a key cooling system.

Exelon Corp's 43-year-old Oyster Creek plant in New Jersey remains on "alert" status, the U.S. Nuclear Regulatory Commission (NRC) said early Tuesday. It is only the third time this year that the second-lowest of four emergency action levels was triggered.

"Oyster Creek is still in an alert but may be getting out of it as long as water levels continue to drop," NRC spokesman Neil Sheehan told Reuters.

The alert came after water levels at the plant rose more than 6.5 feet above normal, potentially affecting the "water intake structure" that pumps cooling water through the plant.

Those pumps are not essential to keep the reactor cool since the plant has been shut for planned refueling since October 22. Exelon however was concerned that if the water rose over 7 feet it could submerge the service water pump motor that is used to cool the water in the spent fuel pool, potentially forcing it to use emergency water supplies from the in-house fire suppression system to keep the rods from overheating.

Exelon also moved a portable pump to the intake structure as a precaution in case it was needed to pump cooling water.

The water levels reached a peak of 7.4 feet -- apparently above the threshold -- but the pump motors did not flood, Sheehan said. As of 11 a.m. EDT Tuesday the water level was down to 5.8 feet, with the next high tide at 11:45 a.m.

"They need the water level to stay below 6 feet for a while to exit the alert," Sheehan said, noting when the water level falls below 4.5 feet, the plant could exit the unusual event.

An unusual event is the lowest of the NRC's emergency action levels.

Exelon said in a statement that there was no danger to equipment and no threat to public health or safety.

"Right now there's no imminent threat of releases. There's no protective actions around the plant," Federal Emergency Management Agency Administrator Craig Fugate said on the Today Show.

"Some of these reporting requirements are due though to the severity of the storms. That they have to make these notifications based upon conditions, that does not mean that they are in an imminent threat at the plant," Fugate said.

The incident at Oyster Creek, which is about 60 miles east of Philadelphia on the New Jersey Coast, came as Sandy made landfall as the largest Atlantic storm ever, bringing up to 90 mile per hour (mph) winds and 13-foot storm surges in the biggest test of the industry's emergency preparedness since the Fukushima disaster in Japan a year and a half ago.

Despite the alert -- which is a serious but not catastrophic event that signals a "potential substantial degradation in the level of safety" -- the U.S. nuclear industry was broadly seen having passed the test. About a dozen alerts have been issued in the past four years, according to NRC press releases.

On Tuesday morning, the NRC said that Entergy Corp's Indian Point 3 automatically tripped offline at about 10:41 p.m. last night due to fluctuations in the power grid caused by the storm, while Public Service Enterprise Group Inc shut Unit 1 at Salem in New Jersey at 1:10 a.m. due to a loss of "condenser circulators" due to the storm surge and debris.

SPENT FUEL

The relatively small 636-megawatt (MW) Oyster Creek plant earlier experienced a "power disruption" at its switch yard, causing two backup diesel generators to kick in and maintain a stable source of power, Exelon said.

The NRC spokesman said the company could use water from a fire suppression system or a portable pump to cool the pool if necessary. The used uranium rods in the pool could cause the water to boil in about 25 hours without additional coolant; in an extreme scenario the rods could overheat, risking the eventual release of radiation.

The concerns over the status of the spent fuel pool at Oyster Creek was reminiscent of the fears that followed the Fukushima disaster last year, when helicopters and fire hoses were enlisted to ensure the pools remained filled with fresh, cool water. The nuclear industry has said that the spent fuel rods at Fukushima were never exposed to the air.

Nuclear plants must store the spent uranium fuel rods for at least five years in order to cool them sufficiently before they can be moved to dry cask storage containers.

Exelon spokesman David Tillman said Monday night the plant has "multiple and redundant" sources of cooling for the spent fuel pool.

The plant uses pumps to take in external water that circulates through a heat exchanger used to cool the internal water that surrounds the rods, keeping them from overheating.

Among other units, Constellation Energy Nuclear Group's 630-MW Nine Mile Point 1 nuclear power reactor in upstate New York did shut due to a problem putting power onto the grid, although it was not clear whether the trouble was related to the storm.

In addition, Sandy caused power reductions at both units at Exelon's Limerick nuclear plant in Pennsylvania and one unit at Dominion's Millstone plant in Connecticut.

David Letterman didn't get many laughs for his monologue Monday: The "Late Show" host told his jokes to a mostly empty Ed Sullivan Theater.

Jimmy Fallon taped "Late Night" without an audience, too, as Hurricane Sandy disrupted a wide swath of the New York area's bustling entertainment industry.

City officials said that all film permits for Monday and Tuesday were revoked because of the hurricane and associated safety precautions.

"There will be no city authorized outdoor filming within the five boroughs," read a statement Monday from the Mayor's Office of Film, Theatre and Broadcasting.

Production on at least nine shows was affected, including "Gossip Girl," ''Person of Interest" and "Elementary," according to the Los Angeles Times. "The Daily Show" and "The Colbert Report" also took Monday night off, and each of Broadway's 40 theaters will be dark through Tuesday.

The storm forced Focus Features to cancel the New York premiere of "Anna Karenina," which was set for Tuesday. A studio spokeswoman said they will reschedule.

Concerts planned for New York and New Jersey were canceled or postponed, including performances by Journey, comedian Louis C.K., and the "Freedom to Love Now" show featuring Rufus Wainwright and fun., which is now set for spring of 2013.

Radio City Music Hall was shuttered until Wednesday. Carnegie Hall postponed concerts planned for Tuesday. Off-Broadway shows and even national tours of Broadway shows like "Anything Goes," currently in Wilmington, Del., closed their doors.

It was the most detrimental storm for the theater community since the threat of Hurricane Irene in late August 2011 prompted producers to cancel matinee and evening performances on both a Saturday and Sunday.

East Coast charity galas were also canceled, and fashion designer Prabal Gurung postponed the unveiling of his anticipated collection for Target until next week.

Some network schedules were shaken up by the widespread power outages cutting into the available TV audience in the East.

CBS planned to air repeats instead of new episodes Monday of "How I Met Your Mother," ''Partners," ''2 Broke Girls" and "Mike & Molly." A CBS News special on Sandy was to follow at 10 p.m. EDT, pre-empting "Hawaii Five-O."

The CW also was set to air repeats of "90201" and "Gossip Girl."

ABC said it was sticking to its schedule of new episodes of "Dancing With the Stars" and "Castle." Fox was to air a previously scheduled rerun of "The X Factor," which had been planned if the San Francisco-Detroit World Series ended, as it did, in four games.

NBC did not immediately respond concerning the status of its Monday schedule, which included "The Voice" and "Revolution."

"Good Morning America," the "Today" show and "CBS This Morning" were expected to air live as usual Tuesday with extensive storm coverage.

Sandy took a toll on the movie box office even before it made landfall. Ticket sales were down more than 11 percent compared to the same weekend last year, said Paul Dergarabedian, box-office analyst for Hollywood.com. This weekend's top film, "Argo," collected $12.1 million. The top film during the same weekend in 2011, "Puss in Boots," earned $34 million.

World Series news: Security in Detroit, a symphonic wager, and all those Venezuelans

The San Francisco Giants and the Detroit Tigers are back at it Saturday in the 2012 World Series. The Giants lead the Tigers two-zip, but don't count Detroit out ??especially in their home park.

The Giants' extended family lingered on the field long after the final out, posing for pictures and savoring the win. Cars honked in the streets outside AT&T Park and fans celebrated in the bars.

San Francisco is halfway to the?World?Series?title, not all the way there. Yet, after two days of beneficial bounces and pivotal plays that went their way, the Giants and their supporters seem to think the team's second championship in three years is only a couple of days away.

"When things are going well," Marco Scutaro said, "things are bouncing your way."

Madison Bumgarner pitched two-hit ball over seven innings, the Giants threw out a runner at the plate and then took advantage of a bunt that stayed fair to push across the go-ahead run in a 2-0 win over the Detroit Tigers on Thursday night that gave them a?World?Series?edge.

"It's a lot less stressful for sure," Bumgarner said, "but at the same time I don't think we can stop pushing or we're going to find ourselves in the same spot we've been in in the last two?series."

And that would be on the brink of elimination.

San Francisco lost its first two games against Cincinnati, then became the first baseball team to overcome an 0-2 deficit in a best-of-five?series?by winning three straight on the road. Then the Giants fell into 1-3 hole against St. Louis before rallying to reach the?World?Series.

"It's great to get off to a good start," Giants manager Bruce Bochy said. "We don't, believe me, take anything for granted."

Detroit police gear up for World Series crowds?

DETROIT

Police in Detroit have a plan to make sure the area around Comerica Park is safe for the thousands of baseball fans coming downtown to watch the World Series. But city officials are not tipping their hand on what they're going to do.

Interim Police Chief Chester Logan told reporters Friday that more than 20 law enforcement agencies will work with his officers when the Tigers host the San Francisco Giants beginning Saturday in Detroit for game 3 of the best-of-seven series.

Logan said "close to 100,000 people" will be downtown Sunday for the Detroit Lions afternoon football game at Ford Field and game 4 of the World Series that night.

He also said no officers would be pulled from Detroit neighborhoods for patrols in and around downtown.

Invisible's 'The New Obsolete' showcases selfconstructed instruments, touts a typewriterdriven piano

If you're hip to repurposing old tech for new inventions, Invisible is right up your alley. The Greensboro-based unit calls themselves a "mechanical music museum" and "a reverse engineered folk science daydream" when describing their elaborate set of sound-making contraptions and recycled video equipment. The outfit's effort The New Obsolete was part of the Moogfest happenings this weekend, and our curiosity was immediately piqued. This particular performance is labeled as "an exploded view of the strange romance between humans and technology."

Among all of the self-constructed instruments is the Selectric Piano: a typewriter that uses both computer and piano parts to control a keyboard. Each keystroke by the typist corresponds to a note added to collective soundscape and a mounted video camera allows the audience to keep tabs on the textual component. The project also showcases an object known as Elsewhere's Roof. The device controls a set of drum and percussion tools with water dropping into a few rather hi-tech Mason jars. In addition to arsenal of noise makers, multi-channel video and library of collected audio (via tape decks and turntables, of course) rounds out the lot. We were able to catch one of the stellar showings, so hit the gallery below for a look at the wares while a snippet of the action awaits beyond the break.

New flexible sports cap could bring better head impact analysis to the game

While the whack of two helmets might be an unavoidable part of some high intensity sports, knowing a little more about what's going on during those impacts can mean the difference between a time out, and time in hospital. Reebok-CCM Hockey and electronics form MC10 have just announced that they are developing a wearable cap that will register the strength and severity of head impacts during games. The project is actually aimed at all sports and age-groups, and uses high-performance electronics reshaped into an ultra-thin, breathable, flexible system that technology partner, MC10, expects to also be much more affordable. The cap will allow quick analysis through the use of different colored readouts, illustrating the strength of impact. The product won't be commercially available until next year however, but we're already thinking of potential worthy collaborations.

The companion app branches out from the earlier My Xbox Live app for Android and allows direct control of some console features from Google-powered gear. While Windows Phone users will find the tightest integration of all, the Android port still lets users steer through the Dashboard, input text, browse the web (with zooming), search for local content, and control media playback. Players can still chat with their friends and modify their Xbox profile if they're not planning to lurk around the system all day. The interface is strikingly similar to past releases (including Android) and still demands at least an 800 x 480 screen for admission -- sorry, Wildfire S fans.
